Moccasin Creek Off Road Park Pet Release & Waiver 

I acknowledge that I am solely responsible for managing my pet while on Property at Moccasin Creek ORP and that I am solely responsible for 
any damage, harm or loss to Moccasin Creek ORP or any other person caused by my pet or in any way arising out of my pet’s presence while on property. Therefore, I agree to fully reimburse Moccasin Creek ORP for the cost of any repair, cleaning, and/or other unforeseen costs related in any way to the on property presence of my pet. 
I certify that my pet has not harmed or shown aggression or threatening behavior toward any person or other animal. I acknowledge that I am solely responsible for my pet’s health, well-being, and veterinary care (including emergency care) if required, during my pet’s presence while on property. 
I hereby certify that my pet is up-to-date on all vaccinations, healthy, free of illness, and has not been in the presence of any contagious disease/ illness/condition, or ill with any communicable disease/illness/condition, within the past six months. 
I hereby expressly agree that I have reviewed and fully accept the terms of Moccasin Creek ORPs Pet Policy and will obey all rules, policies, and instructions set forth by Moccasin Creek ORP and its employees concerning my pet. I understand that Moccasin Creek ORP may, at any time, and for any reason, and in its sole discretion, refuse my pet on property and/or revoke my permission to carry my pet. I also assume all risk of loss and harm to me, my pet, and my property, from whatever source or sources, which may in any way relate to the on property presence of my pet, regardless of any fault, whether it be on the part of Moccasin Creek ORP, its employees, agents, servants, contractors, subcontractors and/or representatives, myself or any other person or entity. 
By executing this Agreement, I knowingly and fully release, indemnify, defend, and save harmless Moccasin Creek ORP, its officers, directors, employees, agents, contractors, subcontractors, insurers, indemnitors, indemnitees, and representatives of and from any and all claims, damages, and losses, including, but not limited to, any and all claims for injury and/or death, loss of or damage to property, and all other losses suffered by Moccasin Creek ORP, myself, and/or any third party which may in any way relate directly or indirectly to my pet, irrespective of any negligence or other fault of Moccasin Creek ORP, its employees, agents, contractors or any party whatsoever. 

I FURTHER ACKNOWLEDGE THAT THIS IS A FULL AND COMPLETE RELEASE WITH RESPECT TO ANY CLAIM ARISING OUT OF 
THE ON PROPERTY PRESENCE OF MY PET. 
This Agreement shall be construed according to the laws of Georgia. I have carefully read and fully understand and accept the provisions of this Agreement and have signed it as my own free act, of my own will and accord. (This form is an digital  agreement, and by adding my acknowledgement and agreement to the following form  may allow Moccasin Creek ORP to permit my pet on property) 

Rules of pets on property at Moccasin Creek ORP 

Pets At MCORP 
Pets must have an entry ticket 
Pets must be checked in at gate entry 
Pets must have a pet waiver prior to entrance 
Pets without a collar band, owner will be asked to leave 
Pets must be kept in an air controlled camper. No tents, trucks or cars. 
Pets must be leashed at all times 
Pets are not allowed to ride in/on ATV, SXS, Golf Carts 
Pets cannot be outside of campers except for bathroom walks 
Owners must clean up after pet 
Non compliance with the pet rules will result in ejection from the park 

Pets are limited to 2 per camper 
There is a 20 pet limit at the park 
Some events will not be available for pet entry 

Pet Fee 
$10 per pet, 2 pet maximum per camper 

Service Animals
Per the ADA, Service animals are defined as dogs that are individually trained to do work or perform tasks for people with disabilities.
A service animal must be under the control of its handler. Under the ADA, service animals must be harnessed, leashed, or tethered, unless the individual’s disability prevents using these devices or these devices interfere with the service animal’s safe, effective performance of tasks.

Service animals are welcome at the park
Service animals must have a waiver
Service animals must be checked in at entry for entry tag 
Service animals must be kept in air controlled environment 
Service animals must be leashed at all times 
Service animals must be kept under full control when in vehicle, on trails 
Service animals require no entry fee, but must be checked in at entry for entry band 
Support pets are NOT recognized by the ADA, and are NOT eligible for entry as service animals

I have read the rules and policies of the Moccasin Creek Off Road Park Pet Release & Waiver . I understand and agree to the rules and policies. By filling out the Pet Waiver (linked below) and Agreement  prior to the attending event, I agree to uphold the rules and pet policies of the park defined by the pet waiver.
